Surgeons should take care when treating ACL injuries in young patients to prevent long-term complications like early onset osteoarthritis, according to a literature review.“There are currently numerous safe and effective surgical techniques to reconstruct the ACL in the skeletally immature sportsperson to restore stability and forestall the early progression towards meniscal and chondral pathologies,” Jeremy S. Frank, MD, pediatric orthopedic surgeon from the Department of Pediatric Orthopaedics and Sports Medicine at the Joe DiMaggio Children’s Hospital in Hollywood, Fla., stated in an AAOS press release.

Platelet-rich plasma did not significantly effectively prevent tunnel widening after ACL reconstruction based on results of this randomized clinical trial.The investigators introduced platelet-rich plasma (PRP) perioperatively in 25 patients undergoing ACL reconstruction with a hamstring graft and compared the results to 25 control patients having the same surgery but without added PRP, according to the abstract.

In this study, return to active duty in young military personnel after treatment of acromioclavicular joint dislocation increases when the dislocation is treated with coracoclavicular ligament reconstruction using two clavicular bone tunnels.“Medial tunnel placement is a significant factor in risk for early failures when performing anatomic [coracoclavicular] CC ligament reconstructions. Preoperative templating is recommended to evaluate optimal placement of the clavicular bone tunnels,” Jay B. Cook, MD, and colleagues wrote in the study abstract. “Placement of the conoid tunnel at 25% of the clavicular length from the lateral border of the clavicle is associated with a lower rate of lost reduction and a higher rate of return to military duty.”

On Feb. 1, 2013, the Centers for Medicare and Medicaid Services (CMS) issued its long-awaited final rule regarding the implementation of the physician payment sunshine provisions of the Affordable Care Act. The final rule does not create any new prohibitions on payments to physicians and teaching hospitals, but rather sets out new dates and requirements for manufacturers and other entities to collect data about their payments to physicians and other “covered recipients” and submit that information to CMS. Applicable manufacturers and group purchasing organizations (GPOs) must begin to collect this data on Aug. 1, 2013 and report the data to CMS by March 31, 2014.

ACL reconstruction with hamstring graft shows good long-term results

Arthroscopic ACL reconstruction with four-strand semitendinosus tendon yielded good short and long-term outcomes and patient satisfaction, according to results of this study.Researchers measured the short-term outcomes of 98 patients 2 years after ACL reconstruction and 52 patients an average of 10.2 years after ACL reconstruction. Mean patient age was 40.4 years. They included patients with isolated ACL ruptures, reconstruction with semitendinosus grafts and patients who had no cartilage alterations or meniscal lesions. Outcome measures included the International Knee Documentation Committee (IKDC), Tegner and Lysholm scores. They classified osteoarthritis (OA) degenerative change using the Jäger-Wirth score and conducted instrumental stability testing using the KT1000 arthrometer (MEDmetric Corporation, San Diego).
